Volunteer Form

Volunteers play a key role in Milpitas Unified School District. They provide essential support to our public schools by mentoring and tutoring students, helping in classrooms and school activities, and helping raise much needed for schools. To join our district, please complete our Volunteer Instructions and Form. Forms take from 2 to 4 weeks to process, so submit your forms quickly.

Student Community Service Form

Engaging in community service provides students with the opportunity to become active members of their community and has a lasting, positive impact on society at large. Community service or volunteerism enables students to acquire life skills and knowledge, as well as provide a service to those who need it most. Your student must complete our Community Service form and turn it in to their Social Studies teacher.

Community Service Requirements

7th & 8th grade Community Service

Students will be awarded for serving the community, but they must seek approval from their social studies teacher and submit their hours by the first of May.
