Applications for free and reduced priced meals are also available in the school office and may be submitted at anytime during a school year. Please turn in all applications to the school office.

Meal Application Form & Income Survey

Student Nutrition Services will no longer collect meal applications in School Year 2023-2024.  Regardless of individual eligibility, all enrolled students will continue to receive breakfast and lunch at no cost. 

Household income information is very important to the District in order to receive school funding. Therefore, the District will be collecting household income in Aeries.  We request that parents complete the survey for each child in Aeries by September 30, 2024. 

 

Meal Application Accepted for Sunnyhills Child Development Center

Meal Application will be collected for Sunnyhills CDC program only. Paper applications are available at the school office.  If you need further assistance, please contact us at Student Nutrition Services, 1585 Roger St, Milpitas, CA 95035, 408-635-2894, EXT 6506.

 

FAIR HEARING: If you do not agree with the school's decision regarding your application’s determination or the result of verification, you may discuss it with the hearing official.
